Protocols for Affiliate Reimbursement for Education Classes
Here’s how it all works! This is where you’ll find the protocols, process, and payment schedule to make sure your affiliate gets rewarded for your hard work offering education classes in your community.
Affiliate Reimbursement Checklist
This is the tool that we use to track the submission of class paperwork as it arrives in the office. It does not need to be sent in. It is presented here as an optional tool for WHM class teachers and/or affiliate leaders to help keep track of paperwork that has been submitted. This is the list of paperwork required to receive reimbursement for classes.
Mini Survey of Diagnoses
A tool for Beginnings class teachers to help keep track of various diagnoses as reported by their students.
Pre-screen Questionaire
Use this tool to help you conduct a phone interview to determine if a potential student is appropriate for your class. A form presented in conversational, script format with fill-in the blanks.Required information to be submitted for affiliate reimbursement. Pre Test 1
This form is found in Workshop 1 of the Beginnings student manual. It is presented here so you can be sure the version you present your students is the most current version. It can also be downloaded if you don't have enough Pre-Needs Assessments available. Required information to be submitted for affiliate reimbursement.
Class Attendance Forms
Sign in sheets to help you track student attendance. There is a separate form designated for each Workshop presented. These completed forms are required to be submitted for affiliate reimbursement.
Class Seating Chart
A teaching tool that allows you to keep track of who is sitting where in your classroom without having to find their name tags.

Guidelines for Providing Child Care
The (almost) complete guide to providing childcare for your class including protocols, payment rates, suggestions, warnings, Incident Report Form and much, much more!
Incident Reporting Form
Grandma always said, "Plan for the worst and hope for the best. " A downloadable MS Word document form used to report emergency incidents (situations where people are physically, emotionally, or mentally harmed). Instructions for this form are included in the Guidelines for Providing Child Care.
Child Care Forms
Use these forms if you are providing child care for your class. Forms include Childcare Consent and Release. Childcare Attendance Form and Childcare Provider Reimbursement Form.
Below are descriptions of the form. All three forms are contained in one PDF file that can be accessed by clicking the "Child Care Forms" link above.
NOTE: If you provide childcare, these completed forms are required to be submitted for affiliate reimbursement.
Child Care Consent and Release Form
This is a legal document designed to protect NAMI Tennessee, the local affiliate and NAMI volunteers. This form must be signed by the adult participating in the NAMI class for each child they place in our care. It is not necessary to have one form per child. Multiple children associated with a single guardian can be listed on a single form. Every child we provide care for must be listed on one of these forms with the signature of their parent or the guardian they are in the care of.
Child Care Attendance Form
This document is an attendance roll of all children that we provide care for. Every child we provide care form must be listed on this form. Use this form every time child care is provided.
Child Care or Training Reimbursement Form
This form must be completed by the childcare provider and a class teacher each time childcare is provided. The childcare provider completes the top portion of the form down to Meeting Site/ Training Name. The class teacher completes the Other Expenses portion by documenting the type of class, class number, class location, and the amount of payment agreed upon. The child care provider completes the document with their signature as requestor.
Post Test 1
This form is found in Workshop 6 of the Beginnings student manual. It is presented here so you can be sure the version you present your students is the most current version. It can also be downloaded if you don't have enough Pre-Needs Assessments available These completed forms are required to be submitted for affiliate reimbursement.
Teacher Evaluation
Allows you to evaluate the class information and make suggestions for improvements and updates. Required information to be submitted for affiliate reimbursement (one from each teacher).
Final Class Report
A tool to help you report the final census of your class. This completed form required to be submitted for affiliate reimbursement.
PROMOTING YOUR CLASS
Class Posters
Posters you can order to help you advertise your class, support group, or education meeting. They are free, and work well on bulletin boards in churches, libraries, grocery stores and other public places. The white space allows you to highlight your local affilate and contact information (click on examples below).
