Protocols for Affiliate Reimbursement for Education Classes Affiliate Reimbursement Checklist This is the tool that we use to track the submission of class paperwork as it arrives in the office. It does not need to be sent in. It is presented here as an optional tool for WHM class teachers and/or affiliate leaders to help keep track of paperwork that has been submitted. This is the list of paperwork required to receive reimbursement for classes. This is a tool developed by one of our teachers to keep track of her "to-do" list. CHILD CARE Guidelines for Providing Child Care The (almost) complete guide to providing childcare for your class including protocols, payment rates, suggestions, warnings, Incident Report Form and much, much more! Grandma always said, "Plan for the worst and hope for the best. " A downloadable MS Word document form used to report emergency incidents (situations where people are physically, emotionally, or mentally harmed). Instructions for this form are included in the Guidelines for Providing Child Care. Child Care Forms NOTE: If you provide childcare, these completed forms are required to be submitted for affiliate reimbursement.
Class Advertising Posters 8 ½ X 14 (Legal size). We pre-print these color posters for you to advertise classes, support groups and events in your community. They are free to you and work well on bulletin boards in churches, libraries, grocery stores and other public places. The white space and perforated pull-off tabs allow customization for local affiliate and contact information. Posters can be printed using any desktop printer. Support Group / Meeting Sign-In Form (pdf) Print this form on 8 ½ X 14 (Legal Size Paper) to gather information from people attending your support group or meeting. Use the MS Word version if you want to edit or personalize the form for your affiliate. Help Save a Tree! Follow this link to fill out the MEETING SUMMARY ON-LINE (live) Use this form to record data from support groups, education and other NAMI meetings. Submit a separate form for each meeting at least monthly. Need a quick and easy way to remind everyone why you’re gathered? Try reading the focus statement at the beginning of every support group. See the examples in pdf and download the Word documents to customize for your support group. Support Group Focus Statement (Family Members Only - pdf) SUPPORT GROUP/MEETING SIGNAGE Affirmations / Meeting Guidelines/ Recovery Process / Problem Management (pdf) These support group signs are presented in an 8 1/2 X 14 (Legal Size) format. They are best printed 40 X 24. To do this you need to find a local print / copy shop with a large format black and white plotter. Most Kinko's will have these. Print out a high quality legal size copy and ask them to blow it up 280%. This will give you the size you need at an affordable price. Many visitors to your support group will be familiar with signs posted in twelve step meetings (Easy Does It, One Day at a Time, etc.). Print these signs on 8 ½ X 14 (Legal Size) card stock and laminate them to add encouraging “Signs of Hope” to your support group meeting. These quotes about hope from famous people will add warmth and encouragement to your meetings.
Make people feel Welcome to your meeting before they enter the doors! Print these signs on 11 X 17 (standard US B size) card stock or paper to tape to the doors or walls of your meeting place. Let first time visitors know they’ve found the right place.
Directions to NAMI Meeting (Generic) (pdf) So they’ve made it to the building where you’re meeting. Now where are you? Leave them a trail of bread crumbs, errr signs. Print these 11 X 17 (standard US B size) signs on your desktop printer or at your local copy shop to help people find where your meeting is in that big old building. Print multiple copies of page one and all the arrows you will need. All left hand turns? No problem. Cut along the dashed line on the arrows page, Velcro the arrow to the bottom of the first page and viola…custom direction signs to your meeting. Change your meeting room? Remove the Velcroed arrow and turn it over to make a new sign pointing the new direction. Need to direct people straight down the hall? Print page two.

Peer-to-Peer Support (pdf) Does your affiliate offer With Hope in Mind and BRIDGES support groups at the same time? Do people get confused about which meeting meets where? Rather than tell people what meeting they should go to, give them enough information so they can decide which meeting is right for them. Print these 11 X 17 (standard US B size)and post them right outside each meeting room to help eliminate confusion. |
