Protocols for Affiliate Reimbursement for Education Classes
Here’s how it all works! This is where you’ll find the protocols, process, and payment schedule to make sure your affiliate gets rewarded for your hard work offering education classes in your community.

Affiliate Reimbursement Checklist

This is the tool that we use to track the submission of class paperwork as it arrives in the office. It does not need to be sent in. It is presented here as an optional tool for WHM class teachers and/or affiliate leaders to help keep track of paperwork that has been submitted. This is the list of paperwork required to receive reimbursement for classes.

Class Task List

This is a tool developed by one of our teachers to keep track of her "to-do" list. Presented in MS Word it can be modified and personalized.

Mini Survey of Diagnosis/Class Composition Report
A tool for WHM class teachers to help keep track of various diagnoses as reported by their students.

Participant Screening
Use this tool to help you conduct a phone interview to determine if a potential student is appropriate for your class. A form presented in conversational, script format with fill-in the blanks. Required information to be submitted for affiliate reimbursement.

Workshop 1 Student Pre-Needs Assessment

This form is found in Workshop 1 of the student manual. It is presented here so you can be sure the version you present your students is the most current version. It can also be downloaded if you don’t have enough Pre-Needs Assessments available. This completed form is required to be submitted for affiliate reimbursement.

Class Attendance Forms
Sign in sheets to help you track student attendance. There is a separate form designated for each Workshop presented. These completed forms are required to be submitted for affiliate reimbursement.

Class Seating Chart
A teaching tool that allows you to keep track of who is sitting where in your classroom without having to find their name tags.

Workshop 8 Student Post-Needs Assessment and Evaluation
This form is found in Workshop 8 of the student manual. It is presented here so you can be sure the version you present your students is the most current version. CHILD CARE

Guidelines for Providing Child Care

The (almost) complete guide to providing childcare for your class including protocols, payment rates, suggestions, warnings, Incident Report Form and much, much more!

Incident Reporting Form

Grandma always said, "Plan for the worst and hope for the best. " A downloadable MS Word document form used to report emergency incidents (situations where people are physically, emotionally, or mentally harmed). Instructions for this form are included in the Guidelines for Providing Child Care.

Child Care Forms
Use these forms if you are providing child care for your class. Forms include Childcare Consent and Release, Childcare Attendance Form and Childcare Provider Reimbursement. Below are descriptions of the form. All three forms are contained in one PDF file that can be accessed by clicking the "Child Care Forms" link above.

NOTE: If you provide childcare, these completed forms are required to be submitted for affiliate reimbursement.

Child Care Consent and Release Form
This is a legal document designed to protect NAMI Tennessee, the local affiliate and NAMI volunteers. This form must be signed by the adult participating in the NAMI class for each child they place in our care. It is not necessary to have one form per child. Multiple children associated with a single guardian can be listed on a single form. Every child we provide care for must be listed on one of these forms with the signature of their parent / guardian they are in the care of.

Child Care Attendance Form
This document is a attendance roll of all children that we provide care for. Every child we provide care form must be listed on this form. Use this form every time child care is provided.

Child Care or Training Reimbursement Form

This form must be completed by the childcare provider and a class teacher each time childcare is provided. The childcare provider completes the top portion of the form down to Meeting Site/ Training Name. The class teacher completes the Other Expenses portion by documenting the type of class, class number, class location, and the amount of payment agreed upon. The child care provider completes the document with their signature as requestor.


Teacher Evaluation
Allows you to evaluate the class information and make suggestions for improvements and updates. This completed form is required to be submitted for affiliate reimbursement.

Final Class Report
A tool to help you report the final census of your class. This completed form is required to be submitted for affiliate reimbursement.

 

PROMOTING YOUR CLASS OR SUPPORT GROUP

Class Advertising Posters
Posters you can order to help advertise your class, support group, or education meeting.

8 ½ X 14 (Legal size). We pre-print these color posters for you to advertise classes, support groups and events in your community. They are free to you and work well on bulletin boards in churches, libraries, grocery stores and other public places.

The white space and perforated pull-off tabs allow customization for local affiliate and contact information. Posters can be printed using any desktop printer. (See Examples)

Print this form on 8 ½ X 14 (Legal Size Paper) to gather information from people attending your support group or meeting. Use the MS Word version if you want to edit or personalize the form for your affiliate.

Support Group / Meeting Summary (pdf)
Use this form to consolidate and record information about people attending your support group or meeting. This completed form is required to be submitted each month for affiliate reimbursement starting October 2009.

Need a quick and easy way to remind everyone why you’re gathered? Try reading the focus statement at the beginning of every support group. See the examples in pdf and download the Word documents to customize for your support group.

Support Group Participant Manual
Print these and have them in chairs before support group participants arrive. Helps first timers know what to expect. Helps long timers remember the format. Don't forget to customize the focus statement (above) and include it in your Support Group Participant Manual. The preferred way to print these is on 11 X 17 paper, double sided, booklet style, stapled in the middle.

SUPPORT GROUP/MEETING SIGNAGE

Affirmations / Meeting Guidelines/ Recovery Process / Problem Management (pdf)

These support group signs are presented in an 8 1/2 X 14 (Legal Size) format.

They are best printed 40 X 24. To do this you need to find a local print / copy shop with a large format black and white plotter. Most Kinko's will have these. Print out a high quality legal size copy and ask them to blow it up 280%. This will give you the size you need at an affordable price.

Signs of Hope (pdf)

Many visitors to your support group will be familiar with signs posted in twelve step meetings (Easy Does It, One Day at a Time, etc.). Print these signs on 8 ½ X 14 (Legal Size) card stock and laminate them to add encouraging “Signs of Hope” to your support group meeting. These quotes about hope from famous people will add warmth and encouragement to your meetings.

Welcome to NAMI (pdf)

Make people feel Welcome to your meeting before they enter the doors! Print these signs on 11 X 17 (standard US B size) card stock or paper to tape to the doors or walls of your meeting place. Let first time visitors know they’ve found the right place.

Directions to NAMI Meeting (Generic) (pdf)

So they’ve made it to the building where you’re meeting. Now where are you?

Leave them a trail of bread crumbs, errr signs. Print these 11 X 17 (standard US B size) signs on your desktop printer or at your local copy shop to help people find where your meeting is in that big old building. Print multiple copies of page one and all the arrows you will need. All left hand turns? No problem. Cut along the dashed line on the arrows page, Velcro the arrow to the bottom of the first page and viola…custom direction signs to your meeting. Change your meeting room? Remove the Velcroed arrow and turn it over to make a new sign pointing the new direction. Need to direct people straight down the hall? Print page two.
